Parent/Guardian Information

WAIVER:

I, the parent/legal guardian, of the player, a minor, agree that I and the player will abide by the rules and regulations of U.S. Club Soccer and U.S. Youth Soccer (USYS), and affiliated organizations, and the Club. I, for myself and minor player, and each of their respective heirs, administrators, assigns and successors, intending to be legally bound, hereby agree to release and indemnify and hold harmless Force North and its officers, board members, employees, agents and representatives, US Club Soccer, USYS and, the owners and operators of the facilities used for the Programs, and their respective directors, officers, employees, agents, and representatives from and against any and all claims, liabilities, damages, or causes of action arising out of or in connection with the player’s participation in the Programs including, in those instances where the Club is negligent, and including without limitation, player’s transportation to/from any Program, which transportation is hereby authorized. I further grant the US Club Soccer, USYS and the Club the right to use player’s name, picture and /or likeness in printed, broadcast and other material concerning the Programs provided such use is related to the player’s status as a participant in the Programs.

By selecting “I agree” below, as the parent/legal guardian of the player I am registering, a minor, I acknowledge and represent that I have read this General Release agreement and fully understand and accept its terms and sign it voluntarily.
